Signing off projects
I think that many organizations tend to fail the importance of signing off projects once they are completed or if a milestone has been met. The clear cut goal should be that every manager or PM should take the time to sign off on projects. This ensures that the project was checked for accuracy and deemed complete by management. I have seen time and time again where project managers do not sign off on projects and they end up having an issue with the project. This simple sign off ensures that everyone is kept honest during the life of a project.
